The prize winner is responsible for any expenses incurred in connection with the prize. No substitution or transfer of prize is permitted. All federal, state and local taxes on the prize are the sole responsibility of the winner. Prize details not set forth herein are the sole discretion of the Company.

Free Year of Botox  (1 Winner) description. Please initial each line item:

  Only one entry per person will be accepted.

  From the date of the drawing and announcement of the winner there is a 1-year expiration date for the prize and its requirements. The last Botox treatment cannot be any later than the date of December 30, 2025. You are subject to the hours and staff availability of Skinlogic Med Spa.

  Prize entails receiving treatments of up to 50 Botox units per individual treatment.

  Botox treatments can be performed once every 3 months or more.

  A minimum of 3 months in between Botox treatments is a requirement of this prize.

  Total Botox treatments received cannot exceed 3 times within the 1-year date of drawing.

  For this prize, the total amount of Botox units administered within the year will not exceed 150 units.

  Subject to Skinlogic Med Spa healthcare provider determines that the Prize winner is an appropriate candidate and that they will not exceed their recommended amounts.

  Prize cannot be transferred, substituted, or botox units banked for future use. This prize has no cash value and cannot be exchanged for cash or other goods and services.

  Botox treatment can only be performed on the face and no other area of the body.

  Subject to Skinlogic Med Spa cancellation policy and fees. $100 fee for not showing up to appointments and must be paid in full before future services can be scheduled. If the prize winner does not show up for more that 1 appointment all future no show appointments can be voided at the discretion of Skinlogic Med Spa.

  I attest that I have read and understand all the foegoing  provisions of this prize prior to its execution and agree to be bound by them.
